    POSITION SUMMARY:

    This position is responsible for providing support and guidance to individuals struggling with addiction issues and/or co-occurring substance abuse and mental health issues. The position will provide advocacy, mentoring, and peer support in the early stages of the recovery process. This position is responsible for working with consumers in a group or individual setting to assist in the building of recovery skills and their application, recovery mentorship, building and utilizing sober support systems, and understanding of the recovery process from the peer perspective.

    The incumbent will assist with the management of daily activities, monitoring the daily schedule, providing medication monitoring, and transportation. This position will work closely with the clinical team and nursing staff to ensure consistency of care. Crisis management and support is also an important function of this position.

    The Gage House is a 15-bed residential facility licensed to concurrently treat substance use disorders and mental health Illness. It is a voluntary treatment program for men and women aged 18 and older. Gage House offers structured treatment activities, individualized to meet the needs of each person entering the program. Individuals can expect to participate in group therapy multiple times per day with a variety of topics and activities. They will also receive regular contact with a psychiatrist and sessions with a counselor. Family participation is encouraged in weekly group and individual sessions as needed. The length of treatment depends on the needs of each person; stays vary in length from 21-90 days as determined by the clinical team. Individuals will also be assessed and linked with community providers as deemed appropriate during their treatment stay.

    MINIMUM QUALIFICATIONS:

    1. High School graduate or General Equivalency Diploma (GED) and be 18 years of age or older.
    2. Must have a minimum of three years in recovery from addiction to alcohol or other drugs and have a strong recovery program that includes natural supports.
    3. Must have Recovery Specialist certification through the Pennsylvania Certification Board.
    4. Must be free of illicit drug use and maintain and promote a clean, sober lifestyle.

    Position Summary:

    The Mental Health Professional (MHP) functions as a clinical member of the multidisciplinary team and provides treatment, rehabilitation, and support services to clients enrolled in the LTSR program. Responsibilities include, but are not limited to, the following: individual and group therapy, case management, medication monitoring, assistance with the activities of daily living (ADL), crisis intervention, psychosocial education, vocational support and supportive counseling.       POSITION SUMMARY:

      The Registered Nurse (RN) functions as a clinical member of the multidisciplinary team and provides recovery oriented treatment, rehabilitation, and support services to clients enrolled in the ACT program. This position is responsible for assessing the physical and mental health needs of each program participant in conjunction with the psychiatrist, which includes planning medical interventions for inclusion in the treatment plan, providing health education, making appropriate referrals to community physicians and maintaining regular contact with the provider pharmacy. The incumbent is also responsible for management of the medication monitoring service which includes maintaining appropriate medication inventory, storage, and security systems; ensuring that all MARs are accurate and up-to-date;, and maintaining up-to-date written and verbal record keeping. Communication with other program staff regarding the clients' medical conditions and medication needs is also critical. The RN, along with every member of the ACT team, must adhere to the fidelity standards, DPW regulations, and the policies and procedures established by the organization.

      The Assertive Community Treatment (ACT) program is for persons with a serious mental illness who have typically not benefited from more traditional outpatient services. This treatment program is designed around the individual needs of the client so that long-term, therapeutic relationships are developed throughout the community.

      The goal is to help clients reduce hospitalizations, increase independent living in the community, build more positive relationships, secure employment and experience an overall greater satisfaction with life.

      The ACT team, consisting of a psychiatrist, therapists, nurses, mental health workers, peer specialists and a team leader, collaborates closely with each consumer to find various services that promote physical and emotional wellness.

      A dedicated ACT Team provides such things as: Service coordination to meet individual needs, Psychiatric services, Individual and group therapy, Substance abuse counseling, Work related services, Daily living skills, Social and interpersonal relationship counseling, Leisure-time skill training, Family education and support, Peer support services.

      The ACT Team provides the intensive rehabilitation, treatment and support services geared toward assisting that client achieve his or her life goals.

      POSITION SUMMARY:

      The Mental Health Professional (MHP) functions as a clinical member of the multidisciplinary team and provides treatment, rehabilitation, and support services to clients enrolled in the ACT program. Responsibilities include, but are not limited to, the following: case management, medication monitoring, assistance with the activities of daily living (ADL), crisis intervention, psychosocial education, vocational support and supportive counseling. The incumbent is expected to provide recovery-oriented interventions according to each client's individual treatment plan and maintain professional relationships with other providers, both internal and external to the program.

      MINIMUM QUALIFICATIONS:

      1. Master's level degree & licensure in related field.
      2. Exceptional communication skills, both written & verbal, organizational skills & the ability to effectively work with a team of other mental health professionals.
      3. Strong clinical skills with working knowledge of the mental health system, psychiatric rehabilitation principles and recovery-oriented practices.
      4. Ability to effectively work with persons with mental illness, developmental disabilities, substance abuse disorders and forensic issues and encourage their participation with comprehensive treatment plans.

        Position Summary

        This position is responsible for storing, ordering, administering and overseeing the self-administration of medications.
