POSITION SUMMARY:

Position responsible for the treatment and care coordination for clients at Gage House. Duties include: level of care assessments, intake/orientation, developing and implementing preliminary and comprehensive treatment plans, as well as providing individual and group counseling to provide clients with the opportunity to work towards treatment goals and explore recovery. Stairways Personal Care Homes are 24-hour, supervised, state-licensed facilities where persons with mental health care needs receive therapeutic support as well as recreational and social opportunities. Residents receive individualized care in safe, home-like environments, recognizing their unique mental health needs. Activities at personal care homes are developed to meet the needs of each resident.     The Adult Residential Treatment Program is a 15-bed facility providing 24-hr, supervised care for up to 30 days. The program provides a safe and supportive setting as an alternative to inpatient mental health hospitalization or as a step-down from inpatient care.
    Not only do individuals in acute psychological distress require kindness and care, but they many need assistance in managing normal everyday routines that, under better circumstances, would not appear daunting at all.

    Treatment is tailored to meet each individual's needs and goals through a holistic, multidisciplinary approach. Services provided can include psychiatry, nursing, individual and group therapy and case management.

POSITION SUMMARY:

This position is responsible for facilitating group and individual behavioral therapy for adults with mental illness and their families. This treatment component is an integral part of a multidisciplinary clinical team that utilizes professional clinical judgment in the use of various therapeutic interventions. Further, the incumbent must maintain a professional relationship with internal program staff and community resource providers as well to ensure quality programming and appropriate aftercare planning. The Registered Nurse (RN) functions as a clinical member of a multidisciplinary team and provides recovery-oriented treatment, rehabilitation, and support services to clients enrolled in the Assertive Community Treatment (ACT) program.     This Nursing position is responsible for the provision and coordination of mental health services for persons incarcerated at the Erie County Prison. The LPN is expected to perform a number of tasks including providing comprehensive mental health assessments, limited individual counseling facilitating linkages to treatment and other related services and prepares for and assists with rounds by psychiatrist and/or Certified Registered Nurse Practitioner (CRNP).     POSITION SUMMARY:

    This Nursing position is within our Mobile Medication Monitoring (MMM) program which is a community based program. The MMM nurse is responsible for providing in-home mobile medication monitoring services to clients in need of skills training, symptom management, and resource coordination. The MMM nurse is responsible for providing mental and physical healthcare education and coordination of medication and related healthcare services; developing and implementing recovery-oriented medication management plans that are comprehensive, accurate and clearly tailored to each client's individual needs; and, conducting ongoing assessments to ensure that goal plans remain relevant, that progress, or the lack thereof, is closely monitored and that modifications can be made as deemed appropriate.
