POSITION SUMMARY:

This position is responsible for supporting the Registered Nurse with assessments of client physical and mental health needs, planning all medical interventions for inclusion in the treatment plan, facilitating health / wellness education groups, medication monitoring, advocacy as appropriate, and the overall surveillance and maintenance of the health needs of clients. The incumbent will also be responsible for maintaining appropriate medication storage/security, including up-to-date MAR, as well as written and verbal communication with other staff regarding clients' medical conditions and medication needs.

MINIMUM QUALIFICATIONS:

  1. Current licensure as Licensed Practical Nurse by State of Pennsylvania; experience working with individuals with mental illness, developmental disabilities, and/or substance abuse disorders preferred.
  2. Exceptional communication skills, both written and verbal, organizational skills and the ability to effectively work with a team.
  3. Strong clinical skills with working knowledge of mental health system and recovery principles.
  4. Ability to effectively work with persons with mental illness, developmental disabilities, and/or substance abuse disorders and encourage their participation with comprehensive treatment plans.
  5. Act 33/34/31 and FBI Clearances
  6. Valid driver's license with access to a vehicle during scheduled work hours.
