Mental Health Worker - Assertive Community Treatment (ACT) program
POSITION SUMMARY:
The Mental Health Worker functions as a clinical member of the multidisciplinary team and is responsible for implementing rehabilitation and support interventions as outlined in the treatment plan for clients enrolled in the ACT program and under the clinical supervision of staff with professional degrees. Responsibilities include, but are not limited to, case management, medication monitoring, assistance with the activities of daily living (ADL), crisis intervention under the clinical supervision of the Team Leader and Psychiatrist, psychosocial education, vocational support and supportive counseling, and consultation with families. The incumbent is expected to provide recovery-oriented interventions according to each client's individual treatment plan and maintain professional relationships with other providers, both internal and external to the program. The Mental Health Worker, along with every member of the ACT team, must adhere to the fidelity standards, DPW regulations, and the policies and procedures established by the organization.
