POSITION SUMMARY:

This position is responsible for enhancing the abilities of clients to lead self-determined lives by providing the necessary support and information for them to implement their rehabilitation / treatment plans in a collaborative and expeditious manner. POSITION SUMMARY:

This position is responsible for supporting the Registered Nurse with assessments of client physical and mental health needs, planning all medical interventions for inclusion in the treatment plan, facilitating health / wellness education groups, medication monitoring, advocacy as appropriate, and the overall surveillance and maintenance of the health needs of clients. Gage House is a 15-bed residential facility licensed to concurrently treat substance use disorders and mental health Illness. It is a voluntary treatment program for men and women aged 18 and older. Gage House offers structured treatment activities, individualized to meet the needs of each person entering the program. Individuals can expect to participate in group therapy multiple times per day with a variety of topics and activities. They will also receive regular contact with a psychiatrist and sessions with a counselor. Family participation is encouraged in weekly group and individual sessions as needed. The length of treatment depends on the needs of each person; stays vary in length from 21-90 days as determined by the clinical team. Individuals will also be assessed and linked with community providers as deemed appropriate during their treatment stay.

This position is a community-based position responsible for providing Psychiatric Rehabilitation Services (PRS) to individuals 18 years and older. The goal of Psychiatric Rehabilitation Services (PRS) is to teach skills that help those diagnosed with mental illness live and work in their communities as independently as possible. The life domains of living, wellness, learning, working, and socializing are the areas that we work within with clients.
