POSITION SUMMARY:

This position is responsible for enhancing the abilities of clients to lead self-determined lives by providing the necessary support and information for them to implement their rehabilitation / treatment plans in a collaborative and expeditious manner. The incumbent also assists clients with scheduled activities, medication monitoring, management of daily tasks, providing transportation when needed, group facilitation, intake information gathering and general collaboration with the Rehabilitation Coordinator, Nurse and other staff. Crisis management and support is also an important function of this position.

MINIMUM QUALIFICATIONS:

  1. High school diploma or equivalent.
  2. Effective communication skills, both written & verbal, organizational skills, knowledge of practical self-care skills & ability to work with minimal supervision.
  3. Ability to effectively work with persons with mental illness and/or developmental disabilities & implement rehabilitation / treatment / recovery plans for their program participation.
  4. Act 33/34/31 and FBI Clearances.
  5. Valid driver's license and access to a vehicle during scheduled work hours.
