POSITION SUMMARY:

This position is responsible for the coordination and implementation of client treatment plans that address the needs and interests of all residents in the RTFA. The incumbent is responsible for assisting in the coordination of referrals, conducting client intakes, gathering all relevant information needed to develop treatment strategies that will help the client in achieving his/her goals, and organizing and coordinating program discharges. Crisis management and resolution are also important functions of this position. The incumbent must maintain a professional relationship with internal program staff and community resource providers which includes facilitation and participation in weekly case review/treatment team meetings.

The Residential Treatment Program for Adults is a 15-bed facility providing 24-hr, supervised care for up to 30 days. The program provides a safe and supportive setting as an alternative to inpatient mental health hospitalization or as a step-down from inpatient care.
Not only do individuals in acute psychological distress require kindness and care, but they many need assistance in managing normal everyday routines that, under better circumstances, would not appear daunting at all.

Treatment is tailored to meet each individual's needs and goals through a holistic, multidisciplinary approach. Services provided can include psychiatry, nursing, individual and group therapy, and case management.

MINIMUM QUALIFICATIONS:

  1. Bachelors level degree in a related field with at least two years of related full-time (or equivalent) experience; or Masters level degree in a related field with at least one year of related full-time (or equivalent) experience.
  2. Bachelors level degree in a related field with at least three years of related full-time (or equivalent) experience; or Masters level degree in a related field with at least two years of related full-time (or equivalent) experience.
  3. Effective communication skills, both written and verbal, organizational skills, knowledge of practical self-care skills and ability to work with minimal supervision.
  4. Ability to effectively work with persons with mental illness and/or developmental disabilities and implement treatment plans for their program participation.
  5. Act 33/34 Clearance and FBI Criminal Background Check.
  6. Valid driver's license with access to a vehicle during scheduled work hours.
